میکرونیدلینگ
میکرونیدلینگ(Percutaneous Collagen Induction Therapy) که به درمان تحریک کلاژن سازی یا PCI نیز معروف است نوعی روش تحریک مکانیکال جهت ترمیم آسیب های ساختاری پوست است که منجر به بازسازی بافت آسیب دیده و تغییر یافته می گردد. هدف این درمان، بهبود ظاهر و عملکرد پوست می‌باشد.
طی فرایند میکرونیدلینگ، با استفاده از سوزن های بسیار ریز، میلیون ها میکروکانال در اپیدرم و درم ایجاد می‌گردد.
وارد کردن سوزن ها به داخل پوست، باعث افزایش انتشار فاکتورهای رشد از فیبروبلاست ها، پلاکت های خونی و بهبود عملکرد فاکتورهای رشد alpha و Beta می‌گردد. این فرایند منجر به افزایش تولید کلاژن و الاستین توسط فیبروبلاست ها می شود. کراتینوسیت ها نیز به محل نیدلینگ در اپیدرم منتقل شده و در آنجا تکثیر می شوند و در نتیجه باعث افزایش ضخامت اپیدرم می گردند.
در این روش نفوذ سوزن به داخل پوست می بایست به حدی باشد که خونریزی های ظریف داخل پوستی ایجاد گردد(pin point bleeding). در میکرونیدلینگ اپیدرم سوراخ می شود اما تخریب نمی شود، بنابراین اپی تلیال در مدت زمان کوتاهی بازسازی شده و در نتیجه دوران نقاهت پس از درمان بسیار کوتاه خواهد بود.

تاریخچه میکرونیدلینگ
تاریخچه بازسازی پوست صورت را میتوان در مصر باستان و استفاده از ماسکهای خراش دهنده(ابریشن) دارای پودر مرمر یافت. اولین کسانی که متوجه مزایای فوق العاده نیدلینگ پوست شدند دکتر اورنتریک و همکارانش در سال 1995بودند. اصطلاح ® Subcision را آنها معرفی کردند.
در این روش سوزن زیرجلدی از طریق ایجاد سوراخی مجاور اسکار، زیر فرورفتگی اسکار یا چروک، فرو برده میشود. عمل آزاد کردن پوست از چسبندگی به بافتهای عمیق، موجب بالا آمدن پوست و ایجاد زخمی کنترل شده می گردد، که در نهایت فرایند بهبود زخم آغاز شده و با تشکیل بافت همبند فرورفتگی اسکار بهبود می یابد.
دکتر آندره کامیراند و همکارانش در مقالهای در سال 1997 با معرفی نیدل ابریشن پیشرفته گام مهم بعدی در باب نیدلینگ پوست را برداشتند. وی به روش Tattoing و با رنگدانه همرنگ پوست نسبت به ترمیم اسکار هیپوکروم صورت اقدام نمود. پس از یک تا دو سال، آنها متوجه شدند با اینکه رنگدانه مصنوعی کاملا محو شده بود، جای آن ملانین طبیعی جایگزین شده و همچنین بافت، ظاهر و رنگ اسکار بسیار بهبود یافته بود.
این اتفاق این فکر را متبادر میکرد که تریپاناسیون (از ریشه یونانی تریپانون: با مته سوراخ کردن) اسکارها به روش tattoing مسئول بهتر شدن و رنگدانه سازی مجدد اسکار است.
آنها به این نتیجه رسیدند که نیدلینگ اسکار بهتنهایی موجب شکستن ساختار کلاژن و تجدید و تحریک ملانین سازی می گردد.
به دنبال کشف دکتر کامیراند، دکتر “دس فرناندس” یک استامپ نیدلینگ برای تحریک کلاژن معرفی کرد. بعدها وی غلتکی (رولر) با 70 سوزن طراحی کرد که استفاده از آن راحتتربود.
